About Our Team
A multidisciplinary team of specialists manages
the care of the patient from the kidney transplant
evaluation visit through the transplant procedure
to postoperative care. Our goal is to provide
the highest quality care to our patients and to
restore each patient to a full and productive
life. We work with patients, their families and
their primary nephrologists to develop the best
treatment and follow-up care plan to meet their
needs.
Our transplant team includes nephrologists, transplant
surgeons, nurse coordinators, social workers,
financial coordinators and discharge planners.

The nephrologist is a medical doctor who
specializes in treating kidney diseases. Penn's

The transplant coordinator is the primary
contact for the patient before and after transplant
surgery. During the evaluation, the coordinator
teaches the patient all about the transplant process
including the evaluation,
the waiting period,
surgery, and follow-up

The social workers provide counseling
for emotional support, problem-solving techniques,
and relaxation therapy for anxiety. The social
workers coordinate a support
group for pre- and post-transplant patients.
They can refer transplant patients to appropriate
community resources and assist them with practical
concerns, such as temporary housing and financial
resources.

The financial coordinator assists patients
with questions and concerns related to health
insurance coverage and kidney transplantation
costs. The financial coordinator will also offer
assistance with obtaining, maintaining and/or
changing insurance plans, Medicare and state assistance.

The discharge planner helps the transplant
patient get ready to leave the hospital following
surgery. This person can help with questions about
obtaining and taking medications after transplant.
