OncoLife Survivorship Care Plan

Oncolink® has created an individualized plan of care based on the Institute of Medicine recommendations for cancer survivors. This free program provides cancer survivors with information regarding the health risks they face as a result of cancer therapies.

It encourages survivors to review the plan with their health care team to further assess their risk and become active participants in their follow-up care.

To access your long term survivorship care plan, go to OncoLife's web site and anonymously enter your type of cancer, chemotherapy, radiation therapy and surgery.
