Penn
Care at Home is the premiere provider of
quality, compassionate nursing care in
the patient's home for Philadelphia
and the surrounding counties. Our home
health nurses are offered a unique situation. They
work as part of a dedicated team of nurses
and caregivers, yet they have the advantage
of working independently, carving out a
schedule that meets both the patient's
needs and their own work/life preferences.
Our
home health nurses are committed to on-going
excellence that allows Penn Care at Home
to treat patients with the most effective
and advanced methods available. The
staff works closely with Penn's nationally
recognized faculty in the areas of cardiology,
oncology, neurology, orthopedics, and infectious
disease. Our nurses are also supported
by a team of social workers and occupational
and physical therapists. |

Infusion Therapy |
Many
patients who require infusion therapy can
receive it where they feel most comfortable – right
in their own homes. This is a wonderful
option for the patient and their family – all
made possible by our clinical staff. As
primary providers of infusion therapy services
to residents of Philadelphia, the surrounding
five counties and New Jersey, our nurses
care for a diverse group of individuals. The
majority of our patients require progressive
therapies at home, and we provide nursing
care that is both high-tech and highly
compassionate.
Our Home Infusion Nurses work directly
with Penn's nationally recognized
faculty to develop protocols for treating
patients with infusion therapy. We
provide a variety of therapies to our patient
population including antibiotics, pain
management, chemotherapy, nutrition, and
factor therapy to name a few.
Our
highly skilled nurses are supported by
an equally skilled team of Pharmacists
who work together to deliver the highest
level of care to our patients. Penn
Home Infusion Therapy nurses are
able to coordinate the patient's care
closely with Penn Care at Home and
Wissahickon Hospice, which allows them
to offer a more comprehensive and efficient
treatment program to the patient. |

Hospice |
As one
of the oldest and largest programs in the
Philadelphia region, Wissahickon
Hospice has been setting the standard of hospice
care since 1982.
As part of the Penn Home Care and Hospice
Services organization, Wissahickon Hospice
cares for patients with a wide variety
of diagnoses including cancer, end-stage
heart disease, end-stage dementia, and
other terminal illnesses. In addition,
Wissahickon Hospice provides bereavement
services after the patient's death,
including a children's bereavement
program.
We provide a supportive environment for
the patient and family to rely on, including
four types of hospice care: basic
daily care, continuous care, nursing home
care including respite care, and inpatient
care. As individuals and as members
of the Wissahickon team, our hospice nurses
are committed to providing the best in
quality services to each and every patient
and family.
Although we employ a team approach to care,
our nurses also practice one-to-one primary
nursing care at its best. By creating a
system of care that honors individuals
who are dying, our nurses enable their
patients to maintain their dignity and
their quality of life. We know that our
nurses also need time for themselves – to
grieve, to regroup, to recharge. We
give you that time. In addition,
Wissahickon Hospice offers a palliative
care program and bereavement.
