| Physical Therapy Appointment
This section pertains to Physical Therapy (Balance
Therapy), if this has been scheduled for you.
Based on your answers to the questions asked of
you when you called for the appointment, you may
be scheduled for Physical Therapy as one of your
first appointments. This is to enable you to begin
receiving treatment as soon as possible. The therapist
will perform an evaluation of your specific balance
or dizziness problem and determine whether you
are likely to benefit from this type of therapy.
Further details regarding how the exercises help
and how often you will need to return will be
established at the evaluation. Usually patients
are given a home program and only one or two follow-up
visits are required.
Attention HMO/POS Patients: Please be
sure your primary physician gives you a separate
referral from any needed for Audiology or doctor
visits. It should be written to "Physical Therapy
- Penn Therapy and Fitness" with our provider
number, which differs depending on insurance.

The procedure code (CPT code) for an initial
evaluation for physical therapy is 97001. If you
are asked for a diagnosis (ICD-9) code, use 781.2.
If you have an HMO, your primary physician is
contracted (this is called capitation) with a
specific facility for Physical Therapy. You must
check with your insurance company to determine
whether you are capitated at Penn Theray and Fitness.
If not, you may be able to request an out-of-network
referral from your physician, who must then get
pre-authorization from the insurance company.
If your primary physician is not a HUP physician,
you probably are not capitated here.
