Privacy Information
New patients are interviewed on the phone by the intake coordinator. If an intake assessment is scheduled, information from this interview is given to the clinicians who meet with the patient. At this evaluation, patients are then registered with the University of Pennsylvania Health System, if they have not previously done so. This registration does not state that you are a cognitive therapy patient.
The report from the intake appointment is given to your assigned therapist. This information is shared only with the director of the center and with a supervisor, if the therapist is in training.
Patients’ confidentiality is as important to your clinicians as it is to you. Information about your treatment that is used for group supervision or case conference will
exclude your name and other identifying information. Records can be
released only with your consent or by special court order.
In general, information that you share with your therapist is confidential, with the following exceptions:
- As required by law, such as a special court order, or when a patient is planning to harm himself or others, or is abusing a child.
- In order for the therapist to receive supervisory consultation.
- In response to the patient's permission to release information to relatives, new therapists, attorneys, insurance companies, etc.
