Eligibility Criteria
Patients must meet specific criteria before
they will be considered for a screening evaluation.
Candidate selection is based on standards established
by the National Institutes of Health.
Patients must have:
- Body-weight greater than 100 pounds or 45
kg above ideal body-weight
- Body Mass Index (BMI) greater than or equal
to 40 or BMI greater than or equal to 35 with
medical complications related to obesity, including
high blood pressure, diabetes, sleep apnea
or gallbladder disease. Estimate your BMI with

- A history of weight loss attempts (supervised
diets and exercise programs). Documentation
of your participation in these programs maybe
required.
- No indication of active alcoholism
or untreated major psychiatric disorder.
- In
addition we request that patients commit
to the following:
- Women of childbearing age
use birth control for 24 months after
surgery and do not plan on becoming pregnant
for two years after surgery.
- Quit smoking
before surgery.
- Begin to take
responsibility for healthy dietary
changes to promote successful weight loss
after surgery.
- Comply with the program's
instructions, appointments and assignments.
Candidates are
evaluated on a case-by-case basis.
Surgeons may use other criteria to aid
them in patient evaluation and selection. Your
surgeon will notify you of any additional
eligibility criteria.
